Interactive Ready for Review for Chapter 30 - Neonatal Emergencies

  • The phase lasts from the first few minutes to the first hours after birth. The %%1%% phase is a period of great physiologic change.

  • More than 90% of newborns breathe spontaneously without any advanced interventions. About 10% of newborns require some assistance, and 1% of newborns require advanced resuscitation techniques.

  • Certain factors such as maternal diabetes, hypertension, and drug use can increase the chance that the newborn will require resuscitation.

  • If you suspect that the newborn will need resuscitation, have all of the equipment open and ready for use.

  • Keep all newborns warm to maintain their body heat.

  • Three criteria are used to assess the need for further resuscitation in newborns--, heart rate, and color.

  • If the newborn's breathing is slow, absent, or inadequate or if the newborn's heart rate is below 100 beats/min, begin ventilations with a BVM device and 100% oxygen at a rate of 40 to 60 breaths/min.

  • If the newborn's condition remains unchanged, consider medications that may help in resuscitation.

  • The is designed to measure the degree of fetal transition or the success of resuscitation. Most healthy newborns will have a score of 7 or 8 at 1 minute and a score of 8 to 10 at 5 minutes. The %%3%% is not used to determine the need for or extent of resuscitation.

  • Special considerations in newborn resuscitation include meconium staining and aspiration, hypothermia, diaphragmatic hernia, twins, newborns with fetal alcohol syndrome, and fetal death.
